Q. A 20 year old male presented to ED with chest tightness and dyspnea. He had wheezing. An ECG was recorded in ED. The clinicians were looking for ischemia and correctly interpreted the ECG as non-ischemic. He was discharged with treatment for asthma.

Answer: Diagnosis. Type 2 Brugada Pattern

Fig 1 shows rSr' with a saddleback pattern in leadsV1-V2, Upright T wave in lead V1, Positive T wave in lead V2, ST segment elevation more than 0.5mm in lead V2, High take-off of the descending limb of the r' more than 2 mm above the isoelectric line and mismatch between QRS duration in leads V1 and V6 (longer in lead V1).
